The Personal Data We Process and What We Do with It
We process any information that you provide in order to respond to your enquiry. If you subsequently make an appointment, your details will be stored on our practice management software. The following categories of personal data may be collected in advance of your attendance: name, company name, address, telephone numbers, e-mail address, date of birth, payment / insurance details, and correspondence regarding your condition. This is stored online, encrypted, on a hosted server on Cloud. Access to this data is limited to licensed computers which are additionally protected by individual sign in details and 10+ alphanumeric passwords.
Sharing Your Personal Data
We will only share your details with your explicit consent. Where third parties are used by us to store your personal data, we ensure they are compliant with the data protection law UK. When you attend for your session, you will be given a consent form to complete with your personal details and to sign to consent to treatment. This form will also ask for your explicit consent to share your details in other circumstances.
Retaining Your Personal Data
The data stored on our practice management system and email server will be kept permanently on our database unless you request to have it removed.
As we process your personal data, you have certain rights. These are a right of access, a right of rectification, a right of erasure and a right to restrict processing. You may request a copy of your data at any time. Please make such a request in writing or by email to the Data Protection Officer, whose details are shown above. Please provide the following information: your name, address, telephone number, email address and details of the information you require.
